Need Creative Lender

Deed shows new owner, has old mortgage from prior owner that needs to be paid off $115 new apprasial $158. Need to pay off mortgage and refi in new name. SISA mid 589. Do I refi as new owner or apply for new mortgage and pay off old mortgage. I am a broker and need wholesale lending.
